Job Description

You will be responsible for collecting data related to the scope of service of Quality and Patient Safety activities at the AKUH. You will assist quality activities under the supervision of Senior Manager, Quality and Patient Safety department.

Specifically, you will be responsible to:
Conduct open and closed medical record reviews of ambulatory, inpatients and review the documentation compliance of health care workers in light of JCIA standards, hospital policies and medical bylaws.
Identify, analyse and compile gaps in documentation and communicates to relevant owners to improve compliance. Develop and disseminate on time reports of all kind of medical record reviews to the relevant stakeholders.
Participate in departmental QIC as a representative of QPS departments and provide assistance wherever required.
Coordinate, compile, analyse institutional dashboard indicators, JCIA library of measures, clinical measures, managerial measures, International Patient Safety Goals, institutional KPIs etc.
Conduct data validation as per institutional criteria for the dashboard indicators as identified by QPS MFT on annual basis.
Collect and compile all the departmental QICs reports on standard format and highlight the areas of improvement in each department and share with concern QIR and QPSD QIC representative of the department.
Assists QPSD manager in timely reviewing of existing multidisciplinary clinical and administrative policies and procedures in the light of current literature.
ollaborate with clinical departments management team and developing, revising forms and patient family education brochures.
Conducts quality and patient safety related audits and spot checks, to ensure provision of safe and high quality care such as International Patient Safety Goals, Patient and Family Education, Code Blue, Crash Carts guided by director and manager of QPSD.
Identifies areas for improvement in effective quality and patient safety related activities and initiate quality or research projects.
Works as resource person for patient/ family education and forms control committee (FCC) and facilitate monthly PFE meeting, keep record of approved, unapproved, under review brochures and forms record meeting minutes
Conduct rounds, spot checks, audits in departments, to monitor compliance of staff with institutional policies, procedures, JCIA and ISO standards.
Participate in internal Quality improvement audits and share audit observations with QPSD manager, director.
Scrutinize Readmission rate of hospital and report it as a Key Performance Indicator of hospital.
Coordinate patient safety rounds with clinical and administrative leaders, keep log of observations /activities and work in coordination with concern department to close the observations.
Conduct, analyse and follow the results of department wise Patient Safety Culture Survey and coordinate with the department stakeholders to improve on the findings.

At least a Graduate degree or equivalent qualification in a related field.
Professional Certification in a relevant field the area of work will be an added advantage.
Minimum 2 years post graduate experience in healthcare setting preferable in any leadership role.
Ability to break down concrete issues into parts and synthesizes succinctly along with collecting and analysing information from a variety of appropriate sources.
Ability to identify client needs and expectations and responds to requests efficiently and effectively while takes action beyond explicit request within established service standards. Able to meet client needs in a respectful, helpful and responsive manner and seek feedback to develop a clear understanding of client needs and outcomes.
Ability to listen actively and objectively without interrupting and present appropriate information in a clear and concise manner, both orally and in writing.
Ability to recognize and act on present issues as well as offer ideas to address current situations or issues. Able to work independently and complete assignments without constant supervision.
Ability to complete tasks in accordance with plans and sets priorities for tasks in order of importance.
Ability to give support to co-workers and works collaboratively rather than competitively as well as share experiences, knowledge and best practices with team members.
